At Millennium Dental Associates, we strive to provide you with the best possible experience from your first visit through to any treatments that might follow. Below you will find an outline of what you can expect as well as any items we may need from you.
One week prior to your first visit please provide the following:
*Note: If you are not computer savvy, please arrive 15 minutes prior to your appointment time, and bring all medications and supplements that you are currently taking with you for review.
During the visit the Doctor will go over your medical history and review your current health concerns. He will then perform a detailed oral exam and complete a series of dental x-rays and intra-oral images.
Please be courteous, and provide at least 24 hours notice for all cancelled appointments.
This helps ensure that we can meet the needs of all of our patients who are waiting to get an appointment. A $75 fee will be charged for late cancellations or no show visits. In order to keep the clinic running on time, if you arrive more than 15 minutes after your scheduled appointment time, you may be asked to reschedule and pay the cancellation fee.
We will complete your insurance forms for you at the conclusion of your visit. You can then submit them for payment. Payment will come to you directly. Millennium Dental Associates is not contracted with any insurance companies and we cannot guarantee our services will be covered by your insurance company. We accept all major credit cards including, Care Credit and Lending Club.
